Is your organization preparing for a major telehealth initiative? Have you already embarked on implementation and run into roadblocks? Have you experienced disappointment in a recent telehealth or technology implementation? We understand! So many telehealth innovations in healthcare fail initially because of the complexities of change in complicated settings. Therefore, these innovations can only be effective in practice if implemented with thoughtful and prepared clinician, program, administrative and executive leadership.

Clinical Transformation applies implementation science, translational science and quality improvement principles to understand, prepare, groom, and nurture a clinical environment for success with any new changes, processes, models, innovations or technologies while assuring minimal disruption to that environment. In this pre-conference workshop, internationally renowned practice transformation experts, Dr. (Prof) Nwando Olayiwola and Candy Magaña will lead participants through a modified version of the Clinical Transformation Specialist Education Program© they have successfully developed at RubiconMD. Participants, ideally those tasked with leading, beginning, adopting, spreading or scaling telehealth innovations in healthcare settings, will acquire or improve skills in leading complex clinical implementations and change.

Nwando Olayiwola,

Family Physician & Chief Clinical Transformation Officer, RubiconMD

Dr. Nwando Olayiwola is a family physician and Chief Clinical Transformation Officer at RubiconMD.  She is currently an Associate Physician and Clinical Instructor at the University of California, San Francisco (UCSF) and an Instructor in the Center for Family & Community Medicine at Columbia University. Previously, she served as the Director of the UCSF Center for Excellence in Primary Care, where she led national practice transformation efforts and primary care research. She has extensive experience as an internationally renowned primary care systems leader and expert in health care redesign, health technology and health equity. Dr. Olayiwola obtained her medical degree from the Ohio State University/ Cleveland Clinic Foundation, completed her residency training in family medicine at Columbia University/New York Presbyterian Hospital, and completed a fellowship in Minority Health Policy and Master in Public Health from Harvard School of Public Health.

Candy Magaña, MPA

Manager of Clinical Transformation, RubiconMD

Candy Magaña, MPA currently serves as the Manager of Clinical Transformation at RubiconMD, which is a health technology start-up dedicated to providing primary care providers with specialty information access through an eConsult platform. Prior to that, Candy was at the Center for Excellence in Primary Care (CEPC) at the University of California, San Francisco (UCSF) where she served as their Manager of Practice Coaching and Training. During her time at CEPC she managed CEPC’s client portfolio, served as CEPC’s primary trainer and most senior practice coach, and collaborated closely with primary care networks and health plans as well as their associated clinics to define and reach practice transformation goals. Prior to CEPC, Candy was a project manager at the San Francisco Health Plan (SFHP) and was the Quality Improvement Program Manager at New York State’s primary care association. She holds a Master of Public Administration in health policy and management from the Robert F. Wagner Graduate School of Public Service at New York University and a Bachelor of Arts degree in public health from the Johns Hopkins University.
